Start with a system map

List the applications, accounts, repositories, and integrations involved. Explain what each one does, who owns it, and how access is managed. Share credentials through an appropriate secure method rather than embedding them in general documentation.

Document workflows and decisions

Describe the business rules, data mappings, scheduled actions, and important assumptions. For AI agents, include information sources, permitted actions, and evaluation examples. Explain where a person reviews output or takes over a task.

Include recovery instructions

Show how to recognize a failed workflow and what to check before retrying it. Identify support contacts and dependencies on third-party services. Provide relevant release and rollback instructions so maintenance does not depend entirely on the original developer’s memory.

Agree on what happens next

Review the handover with the people who will use it. Record known limitations and useful follow-up improvements, and clarify the support arrangements in the project agreement. Ask the receiving team to perform a task

Choose a realistic maintenance task, such as updating a notification template or diagnosing a failed integration. A handover should make it possible to assess a behavior change without relying only on whether the assistant produces a fluent answer.
